Compassionate Support Through Indiana Medicaid Waiver Programs
At Compassionate Hearts Homecare LLC, we provide compassionate home and community-based support services for individuals approved through Indiana Medicaid waiver programs.
Our goal is to help clients remain safe, supported, and as independent as possible in their home and community. We provide care with dignity, respect, patience, and true compassion from the heart.
We support individuals and families receiving services through:
Indiana PathWays for Aging Waiver
Health & Wellness Waiver
Traumatic Brain Injury Waiver
Attendant Care
Hands-on support with daily living needs such as bathing, dressing, grooming, mobility assistance, meal support, and safety supervision.
Home and Community Assistance
Support with daily tasks such as light housekeeping, errands, grocery shopping, meal preparation, appointment support, and community involvement.
Structured Family Caregiving
Support for eligible clients who receive daily care from a family caregiver or live-in caregiver in the home.
Non-Medical Transportation
Reliable transportation to approved appointments, errands, grocery trips, pharmacy visits, and community activities.
Choosing Compassionate Hearts Homecare LLC
If you or your loved one already receives waiver services, you can ask your care manager, case manager, service coordinator, or Medicaid health plan about choosing Compassionate Hearts Homecare LLC as your provider.
You can say:
“I would like to choose Compassionate Hearts Homecare LLC as my waiver service provider.”

Compassionate Hearts Homecare LLC does not determine Medicaid waiver eligibility. Eligibility and service approval are handled through Indiana Medicaid, the waiver program, the care manager, service coordinator, or appropriate state agency.
Services must be approved in the client’s waiver care plan before services can begin.
